Welcome to the Bermuda and Marlowe GP practice. We are proud to offer the highest standard of patient-centred healthcare to all our 14,000 patients. All our staff work in healthcare because we want to help people.
In order to make sure that you see the right clinician each time our receptionists need to know whether you have an urgent medical problem or an ongoing condition. Please help us help you efficiently by answering all their questions. Anything you say is treated confidentially.
The Practice will be moving to electronic prescribing in September. Patients should register with their local Pharmacy as soon as possible in readiness for this system. If you have already registered previously please check that your current Pharmacy is on our system for EPS (not just for collection). This is extremely important as prescriptions may go to the wrong Pharmacy if you do not check.

CHANGES TO HOSPITAL CONTRACTS
TESTS REQUESTED BY THE HOSPITAL:
Please note that if the Hospital requests that you have a test carried out this must be arranged by the Hospital and not the Practice. This includes any requests for blood tests. All results must be obtained from the Hospital and not through the Practice.
This is part of the Hospital contract.
If you have been an inpatient for any reason and require a Fit Note (after 7 days self-certification) this must be provided by the Hospital for the duration of your recovery or until you have a follow up appointment at the Hospital. Please do not contact the Practice to request a Fit Note as these will be declined and you will be directed back to the Hospital.
The Hospital is also responsible for ensuring you have sufficient medication upon discharge (usually 7 days) unless a shorter period is clinically necessary.
MISSED HOSPITAL APPOINTMENTS:
If you are automatically discharged from the Hospital after missing one appointment it is the Hospital's responsibility to organise another appointment for you. Please do not request a re-referral from the Practice as this will be declined and you will be directed to the Hospital.
NOTE: The Hospital can now make an onward referral for patients if it is directly related to the complaint or condition which caused the original referral. There is no need to return to the Practice to request another referral. GP referral is only required for unrelated conditions.

The Bermuda and Marlowe Practice was accredited by the CQC in 2013. More details can be found by visiting the CQC site. Following our recent inspection in July 2016 we were rated as Requires Improvement. The full report is attached.
