Online Services

To access online services such as ordering a repeat prescription, book a telephone triage appointment and to view parts of your medical record please complete an “Online Access” application form.  Please see the “Online Services” section at the bottom of the home page.

Repeat Prescriptions

Sickness Certificates
Please click on the “Sickness Certificates” link in the “Further Information” column on the right of the home page.

Cancelling Appointment
If you need to cancel your appointment please remember to let us know by telephone or use the cancel form.  

Care Data
The roll-out of NHS England’s patient data sharing scheme is to delayed until the autumn. This is to allow time for a clearer explanation of the scheme. You can read more about Care Data and if you would like to opt out of the scheme you will need to complete the opt-out form. Please return this to the surgery.  

Non-English Speakers

These fact sheets have been written to explain the role of UK health services, the National Health Service (NHS), to newly-arrived individuals seeking asylum. They cover issues such as the role of GPs, their function as gatekeepers to the health services, how to register and how to access emergency services.

Special care has been taken to ensure that information is given in clear language, and the content and style has been tested with user groups.
