All travellers who develop relevant symptoms ( for example respiratory symptoms, shortness of breath, cough, fever) however mild, within 14 days of returning or transitioning from mainland China, Thailand, Japan, Republic of Korea, Hong Kong, Taiwan, Singapore, Malaysia or Macau should not attend the practice, but should remain at home and immediately call NHS 111 for advice.

Care Data
The roll-out of NHS England’s patient data sharing scheme is to delayed until the autumn. This is to allow time for a clearer explanation of the scheme. You can read more about Care Data and if you would like to opt out of the scheme you will need to complete the opt-out form. Please return this to the surgery.  

New Patient Registration

If you wish to register with the practice we will require proof of your address. This is because registration is based solely on residency within our catchment area. Please provide any of the following: a TV licence, bank statement, utility bill, documentation from Basingstoke & Deane Borough Council or a housing association/landlord. (The document should show your name and address.) You will also need to bring with you photo I.D. such as a passport or driving licence.

You will need to print and complete an NHS GMS1 Registration form and Medical Questionnaire for New Patients or collect copies from Reception.  Please ensure you complete both forms as they are used for different purposesYour medical records will be obtained from your previous GP by the Primary Care Trust's Registration Department and forwarded to the Bermuda and Marlowe Practice as soon as possible.
